Time Out by Dejal is one of the longest-running macOS break reminder apps. It has been around since the early OS X era and has a loyal user base. It supports two break types: micro-breaks (short pauses every few minutes) and normal breaks (longer intervals). When a break starts, the screen fades and a progress bar appears.

You can skip or postpone any break, and Time Out tracks idle time so it will not interrupt you if you have already been away from your Mac. The break screen supports custom HTML themes, background images, and even embedded YouTube videos. Optional supporter tiers ($3.99 to $14.99, one-time payments) unlock extra themes and activity charts.

Time Out does not read your calendar or detect active calls. If a break fires during a Zoom meeting, you need to skip it manually. The app is closed source.
